Al-Shifa Clinic was established in March 2000 in the low-income neighborhood of Muscoy in San Bernardino, California. It was founded by a small group of concerned
community members who initially donated an average of $2,000 each of their own money to start up the clinic. The mission of the clinic is to provide free, quality,
patient-focused healthcare to all, irrespective of race, religion, ethnicity, or gender and regardless of ability to pay - with the goal of improving the health status of the
community it serves.

Selfless dedication and the spirit of volunteerism are the hallmarks of the physicians, volunteers and the Board of Directors of Al Shifa Clinic. A small group of paid staff
at Al Shifa Clinic exhibit their spirit of dedication through the hard work they put in every day to provide the service to the patients who visit the Clinic.

Al Shifa Clinic is firmly anchored in the ethos of safety, quality care and service, dedication, patient satisfaction, financial transparency and responsibility, and community
help.

Over the years, Al Shifa Clinic has expanded its services. In 2004, a dental unit was started and ten dentists provide their services on a voluntary basis.

In the year 2000, Al Shifa was recognized by the State of California and presented with a letter of commendation by Governor Gray Davis.

Overview of Al Shifa Clinic

Al Shifa Clinic is a non-profit organization governed by a Board of Directors who oversees the multi-discipline institute that has provided much needed services to the
under-served members of the community. A dedicated group of local physicians offer their services on a voluntary basis, with no financial compensation. A small group
of paid support staff help in the operation of the clinic on a daily basis.

The clinic founders located a modular building that the County of San Bernardino was about to dismantle. With the help of funds raised from the community and the seed
money from the founding members, the 5,000-square-feet modular unit was transported to an acre of land in a poor neighborhood of San Bernardino. The unit was
retro-fitted to meet current codes and a parking lot was built around it. The Clinic is approximately 5,000 square feet and includes 16 examination rooms, a medical
library, two waiting rooms, two nurses station, conference room, reception suite, and administrative and physicians offices.

The clinic supports itself mainly through fundraisers organized by the Board of Directors and has also received some grants from the City and County of San Bernardino
and some non-profit foundations such as the California Wellness Foundation. Over the years, the grants from the City and County have dwindled while the Clinic has
expanded its services and increased its patient base. Al Shifa needs more support from other non-profit foundations and health organizations so that it can sustain and
develop its services to an even higher plane.

Community Served by Al Shifa

Al Shifa Clinic provides a "safety net" to the uninsured, indigent, and under-served members of the community who live not only in the surrounding areas, but also come
from neighboring counties, including Los Angeles, Orange and Riverside. Al-Shifa Clinic primarily serves Muscoy and its surrounding areas. The total population of Muscoy
is approximately 42,000 and it is located in San Bernardino County. According to a report, "Overview of the Uninsured San Bernardino County 2002" developed by
Megan Hickey of the Insure The Uninsured Project, November 2002, San Bernardino is geographically the largest county in the United States and has the fourth largest
population in California. She reported that the population consists of Non-Hispanic white (44%); Hispanic (39%); African Americans (9%); Asian & Pacific Islander (5%);
American Indian (1.2%); and others 26%. In 2000, 36% of San Bernardino County residents had incomes below 200% of Federal Poverty Level (FPL). Approximately
54% of the Hispanic population is uninsured.

Additionally, outside of the local population, Al-Shifa is also serving the South Asians and
Middle Eastern communities. According to the 2000 Census, there
are an estimated 7,368 Asian Indians and 8,894 residents of Arab ancestry living in San Bernardino County. Estimates for other South Asians,
specifically Pakistanis, Bangladeshis, Afghanis as well as other Middle Easterners are unaccounted for in the 2000 US Census for San Bernardino.
It is the only community clinic in Muscoy and the only culturally competent clinic to serve South Asians, Middle Eastern and Muslim communities in
the Inland Empire.

The racially and ethnically diverse communities, living in the identified area, are in dire need of accessibility to healthcare services, including
health education and outreach programs, at no or low cost for uninsured, low-income, un-served and underserved members of the community. Al
Shifa Clinic is striving hard to meet the needs and demands of this large population. Sadly, its efforts are constrained by lack of funds.

The Clinic is facing capacity challenges to meet the growing health needs of the culturally and racially diverse population. It is currently serving, on an average, 250
patients a month. It has 4 part-time administrative staff. All physicians work on a voluntary basis.
